Cardiovascular Risk Screening, Metabolic Health Assessments and Lifestyle
Prevention

Prevention starts before symptoms appear.

Many of the most serious long-term conditions, including heart disease, stroke and type 2 diabetes, develop silently over years. By the time symptoms emerge, significant damage may already have occurred. At Halton Pharmacy in Leeds, our Heart and Metabolic Health Hub is designed to help you understand your risks, detect early warning signs, and take meaningful action before a health crisis develops.

No GP referral required. Walk in or book an appointment at Halton Pharmacy, Leeds.

Why Heart and Metabolic Health Matters

Cardiovascular disease remains the leading cause of premature death in the United Kingdom, accounting for around a quarter of all deaths each year. Type 2 diabetes, which shares many of the same risk factors, affects an estimated 4.3 million people in the UK, with a further 850,000 estimated to be living with the condition undiagnosed.

What makes these conditions particularly challenging is that they often develop over many years without causing noticeable symptoms. High blood pressure, elevated cholesterol, and raised blood glucose can all be present for extended periods before a person becomes aware of them, yet each is silently increasing the risk of heart attack, stroke and kidney disease.

At Halton Pharmacy in Leeds, our Heart and Metabolic Health Hub exists to change this. By providing accessible, community-based screening and preventive health support, we aim to help patients across Leeds identify their risks earlier and take steps to reduce them.

The good news is that the risk factors for heart disease, stroke and metabolic conditions are largely modifiable. With early identification, structured support and sustained lifestyle changes, many patients can significantly reduce their long-term cardiovascular risk. Our service is built around that principle.

Understanding Your Cardiovascular and
Metabolic Risk Factors

Cardiovascular and metabolic conditions rarely have a single cause. They typically result from a combination of factors, some inherited, some lifestyle-related, that interact over time to increase overall risk. Understanding which factors apply to you is the first step towards managing them effectively.

The key modifiable and non-modifiable risk factors for heart disease, stroke and type 2 diabetes include:

High Blood Pressure (Hypertension)

One of the most significant cardiovascular risk factors. Often symptomless, it places sustained strain on the heart, arteries, brain and kidneys.

Elevated Cholesterol

High levels of LDL ('bad') cholesterol contribute to the build-up of fatty plaques in artery walls, narrowing blood vessels and increasing the risk of heart attack and stroke.

Elevated Blood Glucose / Diabetes Risk

Pre-diabetes and undiagnosed type 2 diabetes significantly increase cardiovascular risk. Many people are unaware their blood sugar levels are elevated.

Excess Weight and Metabolic Syndrome

Excess body fat, particularly around the abdomen, is associated with elevated blood pressure, abnormal cholesterol levels and insulin resistance.

Physical Inactivity

A sedentary lifestyle is an independent risk factor for heart disease, type 2 diabetes, hypertension and obesity.

Smoking

Tobacco use damages blood vessel walls, accelerates atherosclerosis, reduces HDL cholesterol and significantly increases the risk of heart attack and stroke.

Family History

A first-degree family history of premature cardiovascular disease increases individual risk and should be factored into any cardiovascular risk assessment.

Age and Sex

Cardiovascular risk increases with age. Men generally develop cardiovascular disease earlier than women, though post-menopausal women's risk increases significantly.

Many of these risk factors are present simultaneously and interact with one another. Our Heart and Metabolic Health Hub addresses them as part of a comprehensive, integrated assessment, not in isolation.

Full Assessment

Cardiovascular Risk Screening at Halton
Pharmacy

Our Cardiovascular Risk Screening is a structured, multi-component health assessment designed to estimate your overall cardiovascular risk and identify the specific factors most relevant to your long-term health. This assessment is appropriate for adults who want to understand their cardiovascular risk proactively, as well as those who have been advised to check specific risk factors by their GP or another clinician.

What Your Assessment Includes

Blood Pressure Measurement

Using a clinically validated device to establish your resting blood pressure reading.

Body Mass Index (BMI) Calculation

Height and weight measurement to calculate BMI as a component of metabolic risk assessment.

Waist Circumference

Abdominal adiposity is an independent risk factor for cardiovascular disease and is measured where clinically appropriate.

Cholesterol Testing

Lipid profile testing to identify elevated total cholesterol or adverse cholesterol ratios associated with cardiovascular risk.

HbA1c Blood Glucose Screening

A blood test to screen for elevated average blood glucose, which may indicate pre-diabetes or undiagnosed type 2 diabetes.

Cardiovascular Risk Score Calculation

Using validated risk assessment frameworks to estimate your 10-year risk of a cardiovascular event.

By combining these measurements, we can estimate your 10-year cardiovascular risk, the likelihood of experiencing a cardiovascular event such as a heart attack or stroke within the next decade, and explain clearly what this means for your health and what steps, if any, you should consider.

If our screening identifies any concerns, we will explain our findings clearly and advise you on appropriate next steps. Where a GP review or further clinical investigation is indicated, we will provide a structured referral recommendation. Our pharmacy team does not diagnose medical conditions; where clinical diagnosis is required, we will direct you appropriately.

What a Cardiovascular Risk Score Tells You

A cardiovascular risk score uses your individual measurements and health history to estimate the probability of experiencing a major cardiovascular event over a defined period. It takes into account age, sex, blood pressure, cholesterol levels, smoking status and other factors. This score helps clinicians and patients prioritise preventive interventions, for example, whether lifestyle changes alone are sufficient or whether medication should be discussed with a GP.

A risk score is an evidence-based decision-support tool, not a diagnosis. It is used to guide conversations about prevention, not to alarm patients.

Free Service

Free Blood Pressure Checks in Leeds

High blood pressure, clinically known as hypertension, is one of the most important modifiable risk factors for cardiovascular disease and stroke. It is estimated that approximately one in three adults in the UK has high blood pressure, yet many remain unaware of their condition because hypertension rarely causes noticeable symptoms in its early stages.

Left undetected and unmanaged, persistently elevated blood pressure places sustained stress on the heart muscle, damages arterial walls, reduces kidney function and significantly increases the risk of heart attack, stroke and heart failure over time.

At Halton Pharmacy in Leeds, we offer free blood pressure checks to help identify hypertension early, before lasting damage occurs. You do not need a GP referral or an appointment. Simply visit us at the pharmacy and a trained member of our team will carry out a clinically validated blood pressure measurement and explain your result.

If your blood pressure reading is elevated, we may recommend 24-hour ambulatory blood pressure monitoring (ABPM) to establish a more accurate picture. A single elevated reading in a clinical setting does not necessarily confirm hypertension. ABPM removes these variables by monitoring you in your normal daily environment.

Understanding Your Blood Pressure Reading

Blood pressure is recorded as two numbers: systolic pressure (the pressure when your heart beats) over diastolic pressure (the pressure between beats), measured in millimetres of mercury (mmHg).

Ideal below 120/80 mmHg

Elevated 120-129 / below 80 mmHg

High (Stage 1) 130-139 / 80-89 mmHg

High (Stage 2) 140/90 mmHg or above

Crisis 180/120 mmHg or above

These thresholds are for guidance only. A single reading is not sufficient to diagnose hypertension. If an elevated reading is detected, further assessment will be recommended.

Gold Standard

24-Hour Ambulatory Blood Pressure
Monitoring (ABPM)

A single blood pressure reading, whether taken in a pharmacy, a GP surgery or a hospital clinic, can be influenced by a range of temporary factors that do not reflect a patient’s true underlying blood pressure. Stress, caffeine, recent physical activity, and the clinical environment itself can all cause a transient elevation.

24-hour ambulatory blood pressure monitoring (ABPM) is widely recognised as the gold standard method for confirming a diagnosis of hypertension. By automatically measuring blood pressure at regular intervals throughout the day and night, while the patient goes about their normal daily routine, ABPM provides a comprehensive, real-world picture of blood pressure behaviour that cannot be replicated by clinic-based readings alone.

What to Expect During ABPM

Device Fitting at Halton Pharmacy

A small, portable blood pressure monitor is fitted at the pharmacy. A cuff is placed on your upper arm and connected to a recording device worn on your belt or in a small bag.

Carry Out Your Normal Day

You wear the device for a full 24-hour period while going about your usual activities. The monitor inflates the cuff automatically, typically every 15 to 30 minutes during the day and every 30 to 60 minutes at night.

Return the Device

After 24 hours, return the device to Halton Pharmacy. Your data is downloaded and analysed, and your results are explained to you clearly.

Receive Your Results

Your results are reviewed and explained to you by our pharmacy team. Where the findings indicate that a GP review is required, we will provide clear guidance on next steps.

What ABPM Detects

True Sustained Hypertension

Consistently elevated blood pressure throughout the monitoring period, confirming a diagnosis of hypertension that warrants clinical management.

White Coat Hypertension

Elevated readings in a clinical setting that normalise in the patient's normal environment. This affects a significant proportion of people and can lead to unnecessary treatment if not identified.

Masked Hypertension

Normal clinic readings but elevated blood pressure during everyday activities. Masked hypertension carries significant cardiovascular risk and is difficult to detect without ambulatory monitoring.

Nocturnal Hypertension

Elevated blood pressure during sleep. Blood pressure normally dips during the night; absence of this dip or an elevation during sleep is associated with increased cardiovascular risk.

Blood Testing

Cholesterol and HbA1c Testing in Leeds

Screening for Cardiovascular and Diabetes Risk

Elevated cholesterol and raised blood glucose are two of the most significant, and most commonly undetected, metabolic risk factors for cardiovascular disease. Both can be present for years without causing any noticeable symptoms, yet both are contributing to increased cardiovascular risk throughout that time.

Both tests are performed using a simple fingerprick blood sample at Halton Pharmacy in Leeds, with results available during or shortly after your appointment. Your results will be explained clearly by our pharmacy team, with guidance on what they mean for your cardiovascular risk and what steps, if any, you should consider. Where results suggest the need for further clinical assessment, we will advise you to consult your GP and can provide a summary of your findings to support that conversation.

Prevention

Lifestyle Support and Prevention Services

Screening is only one component of preventing cardiovascular disease and metabolic conditions. The evidence is clear that sustained lifestyle change can significantly reduce cardiovascular risk, and in some cases, reverse the early stages of conditions such as pre-diabetes and metabolic syndrome.

At Halton Pharmacy in Leeds, we take lifestyle support seriously. Rather than simply providing a number and sending patients away, our Heart and Metabolic Health Hub integrates personalised, evidence-based lifestyle guidance into every assessment. We aim to help patients understand not just what their risk is, but what they can practically do to reduce it.

Healthy Diet

Personalised advice on reducing salt intake, increasing fruit and vegetable consumption and improving overall nutritional balance to support blood pressure management.

Physical Activity

Guidance aligned with UK Chief Medical Officer recommendations on physical activity, tailored to individual circumstances and health status.

Weight Management

Support for achieving and maintaining a healthier body weight as part of a comprehensive cardiovascular risk reduction plan.

Smoking Cessation

Smoking is a major independent risk factor for cardiovascular disease. We can provide brief advice, support and signposting to cessation services in Leeds.

Alcohol and Cardiovascular Health

Excessive alcohol consumption raises blood pressure, increases the risk of irregular heart rhythm (atrial fibrillation) and contributes to weight gain. We provide evidence-based guidance on alcohol reduction.

Stress and Cardiovascular Risk

Chronic psychological stress is increasingly recognised as a contributing factor to cardiovascular risk. We can discuss practical stress management strategies and signpost to support services in Leeds.

Lifestyle measures complement, and in many cases reduce the need for, medication. They are most effective when personalised, sustained and supported by clinical guidance rather than generic advice. Our team will work with you to identify the changes most likely to make a meaningful difference to your individual risk profile.

Clinical Service

Weight Management as Part of Heart and Metabolic Health

Excess body weight, particularly central obesity (adiposity around the abdomen), is one of the most significant and modifiable contributors to cardiovascular and metabolic risk. Abdominal adiposity is directly associated with elevated blood pressure, adverse cholesterol profiles, insulin resistance and an increased risk of type 2 diabetes and cardiovascular disease.

At Halton Pharmacy in Leeds, weight management is treated as a clinical service within our Heart and Metabolic Health Hub, not simply a cosmetic concern.

Our approach prioritises long-term health outcomes over rapid short-term results. Weight management medication, where supplied, is always provided as part of a structured clinical programme, never as a standalone product.

When We Refer to Your GP

Halton Pharmacy's Heart and Metabolic Health Hub provides community-level screening, risk assessment and lifestyle support. We are not a substitute for primary medical care, and we do not diagnose medical conditions.
Where our screening identifies results that require clinical review, further investigation or prescription treatment, we will advise you clearly on when and how to consult your GP. In appropriate circumstances, we may provide a written summary of your screening results to support your consultation.

Important: Identifying risk factors does not mean you will develop heart disease or diabetes. It means you can take informed, evidence-based action to reduce your risk. That is exactly what our service supports.

Who Should Visit the Heart and Metabolic Health Hub?

Our service is open to all adult patients in Leeds and the surrounding area. We particularly
encourage the following groups to consider a health check:

Adults aged 40 and over

Cardiovascular risk increases with age. Adults aged 40 and over who have not had a recent cardiovascular risk assessment are encouraged to attend.

People with a family history of heart disease

A first-degree family history of premature cardiovascular disease is a significant independent risk factor that warrants proactive screening.

Individuals living with type 2 diabetes

Diabetes significantly increases cardiovascular risk. Regular screening of blood pressure, cholesterol and overall cardiovascular risk is important.

People who are overweight or living with obesity

Excess body weight, particularly abdominal adiposity, is closely linked to cardiovascular and metabolic risk factors including hypertension, dyslipidaemia and insulin resistance.

Current or former smokers

Smoking is a major cardiovascular risk factor. Former smokers retain elevated risk for a number of years after cessation.

People with known hypertension or elevated cholesterol

Existing cardiovascular risk factors require ongoing monitoring. Regular review helps ensure management strategies remain effective.

Adults with a largely sedentary lifestyle

Physical inactivity is an independent risk factor for cardiovascular disease, obesity and type 2 diabetes.

Those experiencing persistent stress

Chronic psychological stress is increasingly recognised as a contributing factor to cardiovascular risk.

Frequently asked questions

No referral is required for most of our services. You can self-refer for a blood pressure check, cardiovascular risk screening, or lifestyle support consultation at Halton Pharmacy in Leeds. If any findings require medical review or prescribing, we will advise you to consult your GP.

A full cardiovascular risk screening assessment typically takes 20 to 30 minutes. A routine blood pressure check takes considerably less time. Please contact Halton Pharmacy to discuss appointment availability.

A single reading provides a snapshot of your blood pressure at one moment in time and can be affected by temporary factors such as anxiety, caffeine or physical activity. 24-hour ambulatory monitoring records your blood pressure repeatedly across a full day and night, providing a far more reliable picture of your actual blood pressure patterns and enabling more accurate diagnosis and management decisions.

A high cardiovascular risk score does not mean you will develop heart disease. It means that, based on your current risk factors, your calculated probability of a cardiovascular event over the next decade is elevated relative to the general population. Where this is the case, we will explain your results clearly, discuss lifestyle changes that may help reduce your risk, and advise whether you should consult your GP about further assessment or the potential role of preventive medication.

Both tests are performed using a simple fingerprick blood sample, which causes only brief, minimal discomfort. Results are available during or shortly after your appointment.

NHS blood pressure checks are available for eligible patients at Halton Pharmacy. Some other elements of our service may be provided privately. Please contact us for details of current NHS eligibility and private service pricing.

Yes. Our hypertension monitoring and review service supports patients who have already been diagnosed with hypertension. We can provide structured blood pressure reviews, support with medication adherence and lifestyle management, and 24-hour monitoring where clinically indicated to assess the effectiveness of current treatment.

Excess body weight, particularly abdominal adiposity, is directly associated with elevated blood pressure, adverse cholesterol profiles, raised blood glucose, increased cardiovascular risk and a higher likelihood of developing type 2 diabetes. Achieving and maintaining a healthier body weight is one of the most impactful changes a person can make to reduce long-term cardiovascular and metabolic risk.
